Dear Parents and 5th grade students:
TWO MORE DAYS LEFT: *Coming up: Field Trip to UNF Dec. 1. Please remember to send in the field trip slip and $7.50 cash by November 20, which is the deadline for the field trip.

You will find assignments, homework, spelling and vocabulary lists, and forms to help you do your responses to level books. Take the time to explore and help you become a successful 5th grader. Contact me at carnealj@duvalschools.org or Mr. Hamilton at hamiltonr2@duvalschools.org.

Book Report Format
Biography Book Report
If the biography you read did not contain some of this information, please look it up online or in an encyclopedia.
1. Cover page:
Include a drawing of your character, the title of the biography, the author of the biography and your name.
2. Page one:
Tell the date and place where your character was born and raised. Don’t include too many family details or details about habits and hobbies that don’t have anything to do with their later work.
3. Describe the early life of your character. Tell about what kind of person he or she was. What was it about your character that helped him or her to succeed? Did your character know what he or she wanted to become in the future? How did your character prepare for his or her future?
4. Page two:
Tell about the work your character did. Tell why his or her contribution was important, why the work was important. Did he or she invent something or teach others? How did he or she change the world? What lasting effects did your character have on the lives of others?
5. Page three:
What should we all know about your character? How do we benefit today from him or her? What do you think was the most impressive thing about your character?
6. Page four:
Tell how your character has inspired you. In what ways would you like to be him or her?
You will be graded according to the follow criteria:
Clarity and continuity of thoughts
This means that your ideas are presented in logical order and are easy to understand.
Paragraph form This means that you have separate paragraphs for each new idea, you have used a topic sentence for each paragraph, and sentences are complete and include correct punctuation. New paragraphs should be indented. You should use one inch margins.
Spelling Check your spelling before handing in the report.
Neatness Any corrections on the final draft should be done neatly.

Example: randall receive the national honor society award in tallahassee, florida for receiving straight A’s throughout his years at bay street elementary school.
Correction: Randall received the National Honor Society Award in Tallahassee, Florida for receiving straight A’s throughout his years at Bay Street Elementary School.

Rewrite each sentence. Add the correct verb tense and capitalization where it is needed. Rewrite the sentence and underline your corrections.

1. these are marta’s sneakers.
2. we camp at gull island last july.
3. jerome won the good citizen’s award next week.
4. sally wear her favorite hat to meet davy at orange park mall.
Rewrite the following sentence and replace the underlined words with proper nouns and capitalize where needed.
5. my brother is a good musician.

Literature Responses
The second nine weeks the students are using the choices on their "Bingo Genre" page in their reading folder to write their responses.

Also, I am encouraging my students to check out 5 booklets at a time to attain a sticker on their 25 book chart. They are to answer the first 3 questions at the end of each booklet. Please note the booklets correlate with our curriculum in reading, social studies and science; therefore, will enhance their reading strategies that they are tested on during the year and on the FCAT.

Please note: Students are encouraged to use this format at home to do responses since the majority of their reading will be at home. Have your child write a response on notebook paper, you initial the top, and then return to school. Your child will put the response in sequential order in his/her reading folder and get credit for the reading.

Every 60 pages earns a sticker on the "25 Book" Chart. SSS's goal is for 5th graders in Florida to read a million words at their level. Please encourage your child to read at home.

In the event, your child reads a nonfiction book, he/she may follow the above format except the plot (events) will be different. For nonfiction, he/she should write 15 important new facts learned from reading the book. They must be in good sentences and detailed.
